Job summary

Windsor Rehabilitation And Healthcare Center in Windsor, NC, seeks a Speech Therapist to join our long-term care team on a PRN basis. CFY candidates accepted; state license required; ASHA certification preferred. You will assess, treat, and document patient progress per MD orders and facility guidelines.

Qualifications

• Graduate of an accredited university with an MA, or MS in communication disorders recognized by ASHA and state board.
• Holds current license and/or registration in the state.
• Successful completion of CFY.

Responsibilities

• Provide comprehensive speech therapy evaluations as per MD orders.
• Document findings on the standardized evaluation format neatly, accurately and adhering to all guidelines.
• Provide a comprehensive treatment plan including long and short term goals, frequency, duration and treatment modalities, therapeutic interventions, clinical and technical guidelines.
• Ensure MD orders are obtained for evaluations, treatments, recertification's and discharges.
• Provide comprehensive treatment to patients utilizing modalities and modifying patient treatment as indicated and adhering to precautions.
• Document patient’s progress in approved format accurately and timely.
