Job summary

Windsor Rehabilitation And Healthcare Center in Windsor, NC is seeking a Speech Therapist to join our long-term care team. You will evaluate patients, develop and implement comprehensive treatment plans, and collaborate with the interdisciplinary team to meet patient goals.

The role requires ASHA-recognized education, state licensure, and CFY completion, with duties including documentation, MD order management, and occasional coverage of caseloads as needed.

Qualifications

• Graduate with MA/MS in communication disorders recognized by ASHA/state board.
• Holds current license/registration in the state where applicable.
• Successful CFY completion.

Responsibilities

• Provide comprehensive speech therapy evaluations per MD orders.
• Document findings on standardized evaluation formats clearly and accurately.
• Develop a comprehensive treatment plan with long/short-term goals, frequency and modalities.
• Ensure MD orders are obtained for evaluations, treatments, recertifications and discharges.
• Deliver treatment, document progress, and update plans monthly.
• Co-sign CFYs and supervise students per state/federal guidelines.
• Attend facility meetings and participate in rehab team activities.
• Cover other therapists’ caseload during absences and document care plans for MDS.

Qualifications

• Graduate of an accredited university with an MA, or M.S. in communication disorders that is recognized by ASHA and state board.
• Holds current license and/or registration in the state (as applicable).
• Successful completion of CFY.

Job Responsibilities:

• Provide comprehensive speech therapy evaluations as per MD orders.
• Document findings on the standardized evaluation format neatly, accurately and adhering to all guidelines.
• Provide a comprehensive treatment plan including long and short term goals, frequency, duration and treatment modalities, therapeutic interventions, clinical and technical guidelines.
• Ensure MD orders are obtained for evaluations, treatments, recertification's and discharges.
• Provide comprehensive treatment to patients utilizing modalities and modifying patient treatment as indicated and adhering to precautions.
• Document patient’s progress in approved format accurately and timely.
• Document monthly updated treatment plan in standardized format in timely manner.
• Document patient’s discharge and in-service as indicated.
• Document any caregiver or discharge in-services in approved format as indicated.
• Complete any and all other documentation required by Facility.
• Document care plans and information needed for the MDS as deemed necessary by the facility.
• Attend facility and rehab meetings as indicated by supervisor and/or Regional Manager.
• Participate in facility committee meetings as indicated.
• Act as a rehab delegate to the interdisciplinary team.
• Participates in scheduling of patients.
• Perform screens as indicated.
• Complete billing timely and bill patients ethically and accurately for all speech therapy services rendered.
• Cover other therapists’ patient caseload during absence as necessary.
• Supervise junior staff members or students in compliance with State ST Board and Federal regulations and requirements. Co-sign notes of CFY’s and students as indicated as per state and federal guidelines.
• Maintain professional relationship with co-workers, facility staff, patients and patient families.
• Provide in-service education regarding patient care or speech therapy service areas to rehab staff, facility staff and community as needed.
• Comply with all facility policies and procedures and safety guidelines.
• Comply with patient confidentiality and Federal Resident Rights and HIPAA legislation.
• Report to work on time, adhere to scheduled hours and project a professional image at all times.
• Adhere to productivity requirements, complete productivity reports and submit in a timely manner.
• Comply with all Facility Human Resource policies i.e. lunch, overtime, sick and time off notification.
• Maintain a positive work atmosphere by demonstrating and communicating in a professional manner so as to foster positive relations with customers, clients, co-workers and managers.
• Perform any other duties assigned by the department supervisor, lead therapist or Regional Manager.
• Adhere to any and all other Facility written and oral systems and operations.
