• Performs initial therapy evaluations of each patient within 72 hours of receipt of referral.
• Makes opportunities to teach referral sources about determining and recommending appropriate speech and hearing services.
• Communicates regularly with physicians, patients, families, and health care providers to modify plan of care based on patient needs.
• Participates in case coordination with staff nurses and physicians, and documents appropriately in medical records.
• Develops and implements treatment plans based on evaluations.
• Provides clinical follow-up by reporting unfavorable signs and symptoms.
• Manages own schedule to ensure no missed visits or late evaluations due to scheduling errors.
• Instructs other health team personnel and patients.
• Plans for patient discharge.

Qualifications and Skills:

• Current Texas Speech Language Pathologist License.
• Minimum of 1 year clinical experience in home health.
• Current Certification in BSL/CPR.
• Valid Texas Driver's License and Automobile Insurance.
• Knowledge of WellSky EMR software is a plus.

Salary: Pay Per Visit including mileage reimbursement.
