Job summary

Enhance Rehabilitation is seeking a qualified Speech Language Pathologist to join our dynamic team in a SNF setting. You will evaluate and treat patients by following an individualized plan of care after an evaluation.

Strong communication skills are essential as you collaborate with patients, families, and other treatment team members, and you will administer standardized assessments, swallowing evaluations, and develop and monitor treatment plans.

Qualifications

• Experience with administering standardized speech/language assessments, swallowing evaluations, developing treatment plans and monitoring progress.

Responsibilities

• Evaluate and treat patients by following an individualized plan of care after evaluation.
• Provide patient care appropriate for SNF settings with strong communication with patients, families and the treatment team.
• Demonstrate knowledge and skills to support effective patient care and collaborate with other clinicians.

• Strong written, oral and interpersonal skills work well with others and to communicate effectively with patients, families, and other treatment team members when gathering and exchanging patient condition information.
• Experience with administering standardized speech/language assessments, conduct swallowing evaluations, developing treatment plans and monitoring patients' progress.
