Job summary

Enhance Rehabilitation is seeking a qualified Speech Language Pathologist to join our team in a SNF setting. You will evaluate and treat patients by following an individualized plan of care after an initial evaluation.

You will work with patients, families and the treatment team to gather and exchange patient information, administer standardized assessments, conduct swallowing evaluations, and monitor progress as part of comprehensive care.

• Demonstrate the knowledge and skills necessary to provide patient care appropriate in a SNF setting.
• Strong written, oral and interpersonal skills work well with others and to communicate effectively with patients, families, and other treatment team members when gathering and exchanging patient condition information.
• Experience with administering standardized speech/language assessments, conduct swallowing evaluations, developing treatment plans and monitoring patients' progress.
