Position Overview

A Medical Social Worker provides social work services to patients in their residence, following physician orders and care plans, under supervision of the Director of Clinical Services/Nursing Supervisor.

Qualifications

• Master’s or doctoral degree in social work from a Council on Social Work Education-accredited school, with at least one year of healthcare social work experience, or
• Two years of relevant experience (for assistant role).
• Preferred: One year of home care experience.
• Alignment with agency philosophy and goals.
• Initiative and independent judgment.
• Ability to collaborate with individuals and teams to achieve common goals.

Responsibilities

• Adhere to agency policies and procedures.
• Promote Quality Assurance and Performance Improvement (QAPI).
• Serve as a consultant to agency staff.
• Coordinate services with community resources.
• Maintain confidentiality of patient information.
• Act as a resource for employees, patients, physicians, and healthcare providers.
• Communicate effectively and professionally with physicians.
• Maintain punctuality and regular attendance.
• Stay current with Medicare social work service requirements.
• Report incidents or unusual occurrences immediately.
• Participate in continuing education and patient care conferences.
• Engage with community support groups and provide expertise.
• Plan and coordinate social services.
