Job summary

UAB Medicine in Birmingham, Alabama seeks a PRN Licensed Social Worker (LBSW/LMSW) to coordinate the interdisciplinary plan of care for patients from admission to discharge, with weekend and holiday shifts as needed. You will develop care plans across the continuum, assess psychosocial needs, advocate for resources, and support documentation and postdischarge options.

Bachelor’s degree in Social Work is required and Alabama licensure is mandatory.

Qualifications

• Currently a Licensed Bachelor Social Worker (LBSW) or Licensed Master Social Worker (LMSW) by the Alabama State Board of Social Work Examiners required.
• Bachelor's degree in Social Work required.

Responsibilities

• Coordinates the overall interdisciplinary plan of care for patient, from admission to discharge.
• Creates plan for care across the continuum, integrating patient/family preferences and values.
• Serves as a resource to assess, evaluate, recommend and treat legal, neglect, substance abuse, crisis, psychosocial and various adjustment needs.
• Advocates for resources and removal of barriers.
• Maintains ongoing dialog with supervisor and care transition team members to ensure effective implementation and reevaluation of health plan.
• Acts as a resource for adequate medical record documentation, complex level of care recommendations, and services as they relate to diagnoses, and treatment options for postdischarge care.
