The position requires a Bachelor's degree (Master's preferred) and state licensure within 6 months of hire; certification such as Handle with Care within 30 days.

Formación

• Bachelor's degree required; master's preferred.
• Licensure as Social Worker in the state (KY) within 6 months of hire.
• Handle with Care certification within 30 days of hire.

Responsabilidades

• Interview and assess patients and families; determine needed services and interventions.
• Participate in case reviews to evaluate progress and manage discharge planning.
• Consult with healthcare team to monitor treatment plan compliance.
• Assist with discharge planning and connect to transportation, housing, and social services.
• Provide individual, family and group therapy; supervise/train staff and students.
