Job summary

Mercy Fitzgerald Hospital in Darby, PA is seeking a PRN Social Worker to assess and plan interventions helping patients cope with social, emotional, and environmental challenges while supporting families and care teams.

In this role you will conduct patient and family interviews, arrange services, participate in case reviews, assist with discharge planning, and provide individual and group therapy as needed. Strong documentation and interpersonal skills are essential.

Qualifications

• Bachelor's degree required; master's preferred.
• Board Certified Social Worker, or related field (i.e. counseling) in the state of Kentucky (CSW, LPCA, LCSW) within 6 months of hire
• Handle with Care within 30 days of hire.

Responsibilities

• Interviews and assesses patients and/or patient's family, caregivers, and/or legal representatives.
• Determines, prioritizes, provides and/or arranges for needed internal and external services/interventions.
• Participate in case reviews to evaluate case management and progress.
• Consults with healthcare team members to promote, monitor, and evaluate compliance with patient's treatment plan.
• Assists with discharge planning and processes. Identifies appropriate resources, including transportation, housing, healthcare, and social/spiritual services, and provides referrals as part of the discharge plan.
• Supervises and/or trains new staff, students, and interns. Provides individual, family and group therapy.
