Job summary

St. Mary Medical Center in Langhorne, PA is seeking a part-time Social Worker (PRN) to provide psychosocial support to patients and families and to assist with discharge planning within the care team.

You will assess psychosocial needs, coordinate with physicians and nursing staff, facilitate referrals to community resources, and document care plans across adult and aging populations. Qualifications include a Bachelor's Degree in Social Work; a Master's Degree is preferred; hospital-related

• Minimum hospital or health care facility experience is indicated in the posting.

Responsibilities

• Provides direct service to patients and families and works with community agencies, physicians and hospital staff in order to provide psychosocial support and to facilitate discharge planning.
• Modifies approach based on age-specific needs of patient population to include child, adolescent, adult and older adult.
• Demonstrates understanding of patient’s current and prior functional status and psychosocial situation through documented assessment and updates assessment information as new information is obtained.
• Collaborates with physicians, nursing and the Care Management team regarding discharge planning.
• Serves as a resource person to the nursing staff, referred patients and their family members, and physicians.
• Communicates psychosocial information and needs to members of the health care team as appropriate for the continuum of care.

Education

Bachelor's Degree in Social Work

Master's Degree (preferred)
