Moments Hospice is a locally operated hospice dedicated to providing personalized care and treating patients like family. We aim to make patients' final days, weeks, and months as comfortable as possible, fostering a workplace environment where all employees feel part of the Moments family.

We seek exceptional individuals who want to make a meaningful difference in the community and in the lives of others. Our team includes doctors, nurses (NP, LPN, RN), social workers, certified nursing assistants, spiritual counselors, therapists, and volunteers. We are currently looking for a Hospice Licensed Medical Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW) to join us in a PRN/Part-Time role.

Responsibilities:

1. Provide a wide range of psychosocial services to terminally ill patients and their families.
2. Make contact with patient, family, and caregiver within 48 hours of admission.
3. Document psychosocial assessments in the patient's chart within 24 hours of completion.
4. Assess regularly the psychosocial needs of patients and families.
5. Assess pre-bereavement and anticipatory grief needs.
6. Implement the Plan of Care to address social service goals, supportive counseling, problem solving, and community referrals for pre-bereavement and bereavement care.
7. Re-evaluate psychosocial needs periodically.
8. Participate in weekly interdisciplinary team meetings.
9. Assist with funeral arrangements if requested.
10. Provide counseling during terminal illness and at the time of death if requested.

Qualifications:

• Master's Degree in Social Work from an accredited school (Council on Social Work Education).
• Minimum of two years of social work experience in healthcare or social services.
• Current Social Worker license.
• Hospice experience is a plus.
• Knowledge of community resources, including information and referral services and federal/state financial programs.
• Understanding of hospice philosophy and principles of death and dying.
• Strong organizational and communication skills.
• Experience as a Case Worker is desirable.
