Qualifications

• No experience required; on-the-job training provided.
• One year in a healthcare setting is preferred.

Responsibilities

• Assess post-hospital care needs and coordinate with the care manager; ensure transition to the least restrictive environment.
• Refer to acute rehabilitation, extended care facilities, home health agencies, DME providers, and other resources as necessary; address psychosocial needs and crises as needed.
