Job summary

Eden Senior Care’s Fair Oaks Lodge in Wadena, MN is seeking a PRN Speech-Language Pathologist to provide rehabilitation services at one or more sites within the Eden network.

The role requires a Master’s degree in Speech-Language Pathology and an active Minnesota SLP license, with preference for prior long-term care experience. Work is in a skilled nursing setting with flexible, per-diem scheduling and a $750 sign-on bonus.

Qualifications

• Master's degree in Speech-Language Pathology from an accredited program.
• Active Minnesota Speech-Language Pathologist (SLP) license.
• Experience in long-term care or a skilled nursing facility is preferred.

Responsibilities

• Performs therapy interventions utilizing standard therapy techniques and skills as appropriate for the condition of the patient.
• Communicate patient progress or problems to supervisor and other team members; assist with patient scheduling and post charges daily to patient records.
• Instruct patient's family or nursing staff in follow-through programs.
• To evaluate and obtain data necessary for treatment planning and implementation.
• To develop a treatment plan that includes a target list, short-term and long-term goals.
• Instructs patient, family, and staff in appropriate speech-language procedures to allow carryover of therapeutic remediation and goals in daily activities.
