Qualifications

• Active RN state license required.
• Valid driver's license is necessary.

Responsibilities

• Accept client admissions as assigned by Agency.
• Conduct initial home assessments of clients.
• Prepare written plans of care for administration of care.
• Conduct supervisory visits to client's home.
• Coordinate care for clients with staffing coordinators.
