Responsibilities

• Assessment/reassessment of health care needs for patients across age groups.
• Nursing diagnosis and outcomes planning with patient/family involvement.
• Implementation and coordination of care using evidence-based practice.
• Evaluate patient outcomes and discharge needs as per the plan of care.
