HCA Houston Healthcare Southeast in Pasadena, Texas seeks a PRN RN Case Manager to support patients and families through the care journey. You will coordinate services, discharge planning, and develop individualized plans in partnership with physicians, nurses, and care teams.

Qualifications

• Graduate of an accredited diploma, associates, or baccalaureate degree nursing program.
• Bachelor’s Degree in Nursing preferred.
• Current Registered Nurse License in the State of Texas or Multi-State Compact License (Texas RN license within 90 days of hire).
• 2+ years in case management OR 3+ years in clinical nursing.

Responsibilities

• Guide patients and families through orientation, discharge criteria, and rights.
• Coordinate education for patients, families, and caregivers.
• Develop and implement individualized treatment plans reflecting patient needs.
• Complete psychosocial assessments and discharge planning evaluations.
• Collaborate with Rehab Program Director and interdisciplinary team for patient-centered care.
• Promote HCA Healthcare values in every interaction.
