Preferred Home Health Care & Nursing Services is seeking a PRN RN Care Coordinator to manage patient care across acute, chronic, and long-term stages. You will coordinate discharge planning, assess psychosocial status, advocate for patients, and link to resources to ensure seamless transitions and optimal outcomes.

You will collaborate with the care team to develop individualized care plans, address barriers and cultural considerations, and participate in ongoing quality improvement initiatives

Formación

• RN licensure is current and in good standing.
• BLS certification or higher in related fields.

Responsabilidades

• Care coordination and discharge planning for patients.
• Assess psychosocial and functional status of patients.
• Advocate for patients, provide education and connect to resources.
• Utilize management and ensure regulatory compliance.
• Collaborate with healthcare team to monitor and adjust care plans.
• Mentor staff and students; support performance improvement.
