Job summary

CareRing Health is seeking a qualified Registered Nurse to provide comprehensive home health nursing, including initial assessments, care planning, and on-going patient education. You will work with clients and families to ensure outcomes are met in the home setting.

The role requires two years of acute care experience, current RN license, and CPR certification. On-call weekends are included to support admission and coverage needs.

Qualifications

• Must be a graduate from an approved school of nursing.
• Must hold a current RN license.
• CPR certification required.
• Minimum 2 years of acute care experience with heart, lung, and physical assessments.

Responsibilities

• Complete initial and periodic comprehensive assessments for admission and ongoing care.
• Accept admissions and weekend on-call assignments.
• Provide comprehensive nursing care using home assessments and physician-approved care plans.
• Perform treatments ordered by the physician that require specialized skills.
• Monitor signs and symptoms and report to clinical leadership.
• Note changes in client physical or emotional status.
• Assess nutritional needs and dietary requirements.
• Educate patients and families on medications and procedures.

Responsibilities

• Completes initial and periodic comprehensive assessments of clients, caregivers, and home environments for admission to and continued service from the Agency to determine needs, effectiveness of care provided, and achievement of anticipated outcomes. Assessments may include OASIS items at appropriate time points.
• Accepts client admissions as assigned by Agency and weekend on-call assignments.
• Provides comprehensive nursing care through the utilization of the home assessment, nursing diagnoses, and physician‑approved care plans.
• Performs treatment(s) requiring specialized skills ordered by the physician.
• Observes signs and symptoms and reports them to the VP of Clinical Services / Director of Professional Services and/or physician as appropriate.
• Notes changes in physical or emotional status of clients.
• Assesses nutritional needs and diet of clients as ordered by physician.
• Educates patients families on medication and procedures.

Qualifications

• Holds a current Registered Nurse license.
• Is a graduate from an approved school of nursing.
• Has a minimum of two years of experience in an acute setting that requires heart, lung, and physical assessments or has demonstrated ability or education in the same areas.
• Has current CPR certification.

• Has excellent communication, organizational, and human relations skills.
• Is able to function with minimal supervision.
