Job summary

Elara Caring is seeking a Registered Nurse – Home Health to deliver compassionate, high-quality care to patients in their homes across the assigned territory. The role involves conducting initial and ongoing evaluations, developing and updating care plans, and coordinating with the interdisciplinary team to meet patient and family needs.

Qualifications

• Current, unrestricted RN license.
• 1+ year clinical nursing experience.
• Willingness to travel within assigned territory (up to 50%).
• Valid driver’s license, reliable vehicle, and current auto insurance.
• Ability to lift/push/pull up to 50 lbs and assist with patient mobility.
• Home Health, Hospice, or HomeCare HomeBase experience preferred.

Responsibilities
