Job summary

Elara Caring is seeking a Registered Nurse – Home Health to provide compassionate, high-quality care to patients in their homes. You will assess needs, develop and update care plans, and coordinate with an interdisciplinary team to ensure safe, effective home-based care.

The ideal candidate holds a current, unrestricted RN license, has 1+ year of clinical nursing experience, and is willing to travel up to 50%.

Qualifications

• Current, unrestricted RN license.
• 1+ year clinical nursing experience.
• Willingness to travel within assigned territory (up to 50%).
• Valid driver’s license, reliable vehicle, and current auto insurance.
• Ability to lift/push/pull up to 50 lbs and assist with patient mobility.
• Associate’s or Bachelor’s in Nursing preferred.
• Home Health, Hospice, or HomeCare HomeBase experience preferred.

Responsibilities

• Align care with Elara Caring mission and leadership direction.
• Maintain HIPAA confidentiality standards.
• Assess patient and family needs (physical, psychosocial, environmental).
• Manage full caseload including planning, coordinating, and evaluating care plans.
• Perform initial and ongoing patient evaluations and update care plans.
• Communicate changes in patient condition to clinical leadership and care team.
• Revise and coordinate care plans with interdisciplinary team.
