Qualifications

• Treat patients according to the treatment plan developed by the Licensed Physical Therapist.
• Participate in patient plan of care meetings if asked by the Physical Therapist.
• Record treatment and progress notes in compliance with policy.

Responsibilities

• Communicate with the Physical Therapist regarding patient progress.
• Instruct families on maintenance programs under supervision.
• Participate in discharge planning under the supervision of the Physical Therapist.
