Job summary

Potomac Valley Nursing and Rehabilitation Center is seeking a PRN Physical Therapist to support patient rehabilitation in a long-term care setting. You will provide direct care, assess medical status, and contribute to individualized treatment plans for residents.

You will document patient progress, educate patients and families, and collaborate with a multidisciplinary team to ensure high-quality, patient-centered care in a supportive work environment.

Qualifications

• Must have a valid professional license and registration for state of practice.
• Experience with long-term care and/or geriatric care is preferred

• Establish and administer a treatment program with specific goals determined according to the patient’s capacity and tolerance under the direction of the Physician.
• Document and record the patients' condition and educate patients and families in an appropriate physical therapy method.
• Collaborates with all disciplines to provide best patient centered treatment and care.
