Job summary

Paradigm Home Health Companies is seeking a Physical Therapist Assistant (PRN) in the McKinney/Frisco/Allen area. The role involves implementing care plans with the supervising PT and educating patients and families on home safety and equipment use.

You will communicate patient progress to the PT and care team, work independently, and stay current with technology while delivering compassionate home health care in a PRN capacity.

Qualifications

• Associate degree in physical therapy.
• Current BLS/CPR certification.
• Home Health experience.
• Valid Texas driver’s license and auto insurance.
• Knowledge of WellSky EMR software is a plus.

Responsibilities

• Implement plan of care in consultation with the physical therapist and care team.
• Communicate findings or changes to the physical therapist and care team.
• Educate patients and families in home safety and medical equipment safety.
• Be solution-driven, self-motivated, and inspiring to patients to reach goals.
• Work independently without direct oversight.
• Know when to call for support and communicate challenges.
• Familiar and comfortable with technology.
