Associate degree in Physical Therapy is required.
Current BSL/CPR certification.
Home health experience preferred.
Valid Texas driver’s license and automobile insurance required.
Knowledge of WellSky EMR software is a plus.

Responsibilities

Implement plan of care in consultation with the physical therapist and care team.
Communicate findings or changes to the physical therapist and care team.
Educate patients and families in basic home safety and medical equipment safety.
Be solution-driven, self-motivated, and inspire patients to reach goals.
Work independently without direct oversight.
Know when to call for support and communicate challenges.
Be familiar and comfortable with technology.
