Job summary

Stern Therapy Consultants is hiring a PRN Physical Therapist Assistant to support the rehab team at Civita Care Center at Meadowbrook in Granby, CT. You will assist residents with movements needed for daily activities under a Physical Therapist's treatment plan and help them regain mobility.

Responsibilities include carrying out PT plans during sessions, guiding walking, transfers, balance and strengthening exercises, monitoring tolerance, coordinating with nursing, and recording progress after

Qualifications

• Active Connecticut Physical Therapist Assistant license.
• Strong observation, communication, and documentation skills.
• Skilled nursing experience is a plus.

Responsibilities

• Carry out PT treatment plans during scheduled rehab sessions.
• Help residents practice walking, transfers, balance, and strengthening exercises.
• Monitor residents' tolerance and report changes to the supervising PT.
• Coordinate with nursing when residents need support during their recovery.
• Record treatment and progress after each visit.
