Job summary

Qualifications

• BS in Physical Therapy from an accredited program.
• Active Physical Therapy license in state(s) of practice.
• Ongoing continuing education and professional development to maintain licensure.

Responsibilities

• Provide physical therapy services to residents in accordance with established standards and facility procedures.
• Adhere to documentation and billing procedures for physical therapy services.
• Demonstrate sound clinical judgment in evaluation, planning, and follow-up of resident therapy programs.
• Conduct regular screenings to determine need for intervention or treatment.
• Evaluate residents to inform treatment planning and implementation.
• Develop and implement treatment plans with short- and long-term goals.
• Interpret and communicate evaluation findings and recommendations to residents, families, and the interdisciplinary team.
• Complete CMS-ready documentation in the EMR system as required.
