Job summary

Qualifications

• Bachelor's degree in Physical Therapy from an accredited institution.
• Current state licensure as a Physical Therapist.
• Experience in a skilled nursing or long-term care setting preferred but not required.
• Compassionate, patient-focused and detail-oriented with strong organizational skills.

Responsibilities

• Patient Assessment: Evaluate patients' physical conditions, mobility, and functional abilities to develop comprehensive treatment plans.
• Treatment Planning: Create and implement personalized therapy programs to address goals, improve mobility and reduce pain.
• Therapeutic Interventions: Administer therapeutic exercises, manual therapy, and modalities to aid rehabilitation.
• Progress Monitoring: Regularly assess and document patient progress; adjust plans as needed.
• Patient Education: Instruct patients and families on exercises, safety, and home care strategies.
• Collaboration: Work with physicians, nurses, and other professionals for coordinated care.
• Documentation: Maintain timely documentation of evaluations, plans, progress notes, and discharge summaries.
• Compliance: Follow safety, infection control, and facility policies and procedures.
