Job summary

Emerald Healthcare & Rehab Cozad is seeking a motivated Physical Therapist to join our Rehab department on a PRN basis. The therapist will assess residents, design individualized therapy plans, and deliver evidence-based interventions in a skilled nursing setting.

Collaborating with physicians, nurses, and other therapists, you will promote mobility and independence, document progress, and educate families on home exercise programs while adhering to facility policies and safety protocols.

Qualifications

• Education: Doctoral or Master’s degree in Physical Therapy from an accredited program.
• Licensure: Current state licensure as a Physical Therapist.
• Experience: Preferred in skilled nursing or long-term care; not required.
• Attributes: Compassionate, patient-focused, detail-oriented, collaborative.

Responsibilities

• Patient assessments and mobility evaluation to develop treatment plans.
• Plan and implement personalized therapy programs to improve function and reduce pain.
• Administer therapeutic exercises, manual therapy, and modalities.
• Monitor progress and adjust plans to optimize outcomes.
• Educate patients and families on home exercise strategies.
• Collaborate with physicians, nurses, and therapists to coordinate care.
• Document evaluations, plans, progress notes, and discharge summaries.
• Ensure compliance with safety, infection control, and facility policies.
