Job summary

Legacy Healthcare Services, Inc. is seeking a Physical Therapist (PRN) to join our team.

This role focuses on delivering individualized, private rehabilitation sessions to senior residents, with a commitment of at least 8 hours per month to maintain active employment. You will plan treatment programs based on clinical findings, collaborate with a coordinated care team, and participate in conferences to align the plan of care.

Qualifications

• Degree in the relevant discipline required.
• Current Physical Therapy license in state of practice, or ability to obtain prior to employment.
• Excellent interpersonal, fiscal, and clinical skills.
• Computer proficiency required.

Responsibilities

• Provide rehabilitative services to patients under clinical orders as part of a coordinated team.
• Plan initial and subsequent treatment programs based on clinical findings.
• Participate in clinical conferences to align the plan of care for patients.
• Treat residents one-on-one with individualized, function-based sessions.
