Job summary

Pro Rehab is seeking a Physical Therapist to join our team in Eagle, Colorado on a PRN basis. The role emphasizes comprehensive patient assessment, personalized treatment plans, and hands-on therapy to improve functional outcomes.

The ideal candidate will hold a CO-licensed PT credential, have graduated from an accredited program, and possess strong English communication skills to educate patients and collaborate with care teams. Position is on-site at Cassia community locations in Colorado.

Qualifications

• Graduate from an Accredited Physical Therapy Program and hold a bachelor’s degree.
• Current CO Physical Therapist License.
• Knowledge of state, federal, and local Home Health regulations.
• Able to communicate effectively in English, verbally and in writing.

Responsibilities

• Assess & Diagnose: Evaluate patients' physical conditions and history to tailor treatment plans.
• Develop & Implement Treatment Plans: Create rehab programs to address deficits.
• Provide Hands-on Therapy: Perform manual therapy and therapeutic exercises with modalities as needed.
• Educate & Support Patients: Teach exercises, pain management, and fall prevention.
• Monitor Progress & Adjust Plans: Track outcomes and adapt plans accordingly.
• Collaborate with Healthcare Teams: Work with doctors, nurses, and other therapists for holistic care.
