Job summary

Lake City Healthcare and Rehabilitation is seeking a dedicated Physical Therapist to join our rehabilitation team on a PRN basis in Lake City, FL. The PT will provide high-quality therapy services, develop individualized treatment plans, and collaborate with an interdisciplinary team to improve residents’ mobility and independence.

Qualifications

• Bachelor’s Degree in Physical Therapy from an accredited program.
• Current Florida Physical Therapist license and/or registration.
• Experience in skilled nursing, rehabilitation, hospital, or healthcare settings preferred.
• Strong communication, organizational, and clinical skills.
• Ability to work effectively as part of an interdisciplinary team.
• Commitment to providing compassionate, resident-centered care.

Responsibilities

• Plan, develop, organize, implement, evaluate, and direct physical therapy services per regulations and facility guidelines.
• Collaborate with administration, physicians, nursing staff, and interdisciplinary team members.
• Perform comprehensive patient evaluations including ROM, manual muscle testing, gait analysis, and functional assessments.
• Develop and administer individualized PT treatment plans to restore function and relieve pain.
• Monitor resident progress and adjust treatment plans to achieve goals.
• Complete accurate documentation including evaluations, plans of care, and progress reports.
• Maintain professional standards and ensure compliance with policies and regulations.
• Provide guidance to therapy staff and promote collaborative care.
• Participate in interdisciplinary meetings and quality improvement initiatives.
