Qualifications

• Graduate of an accredited Physical Therapy program.
• PT license in state of practice is required.
• New graduates welcome where applicable.
• Experience in SNF, rehabilitation or post-acute care preferred.

Responsibilities

• Evaluate patients with orthopedic, neurological, and mobility-related conditions.
• Develop and implement individualized treatment plans.
• Improve strength, balance, gait, endurance, and mobility.
• Monitor patient progress and modify interventions as needed.
• Complete timely clinical documentation.
• Collaborate with physicians, nurses, therapists and caregivers.
• Educate patients and families on rehab goals and home programs.
