Job summary

HCA Florida Putnam Hospital in Palatka, FL is seeking a Physical Therapist to support PRN coverage. You will evaluate patients, create personalized plans, and deliver interventions to improve mobility and reduce pain.

The role involves documenting outcomes and coordinating with physicians and the care team. As part of a national clinical network, you will mentor students or therapy assistants and contribute to high-quality patient care, with weekend coverage and potential weekday opportunities

Qualifications

• State-licensed physical therapist in Florida.
• Bachelor's degree in a related field.
• BCLS certification required within 30 days of start date.

Responsibilities

• Evaluate patient abilities and develop personalized treatment plans with physicians.
• Deliver evidence-based interventions to restore movement and reduce pain.
• Monitor progress and document outcomes accurately.
• Educate patients and families on exercise, techniques and safety.
• Collaborate across disciplines for cohesive, patient-centered care.
• Lead by example; mentor therapy assistants or students while upholding values.
