Job summary

Concentra, Inc. is seeking a Physical Therapist to deliver high quality rehabilitative services for disabled, injured, and diseased patients in our centers, following our Occupational Medicine rehabilitation philosophy and policies.

You will perform direct patient care, evaluate musculoskeletal, neurological and other conditions, and work with a multidisciplinary team to ensure coordinated, effective treatment plans and documentation.

Qualifications

• Bachelor’s Degree from an accredited Physical Therapy program.
• Masters’ Degree from an accredited Physical Therapy program, preferred.

Responsibilities

• Ensure delivery of high quality patient care services.
• Perform direct patient care, including evaluation of muscle, skeletal, neurological and other physical problems.
• Support and encourage multidisciplinary coordination of treatment.
• Ensure proper documentation of patient care.
• Maintain professional standards through compliance with quality assurance policies and procedures as well as applicable regulations and corporate policies.
• Implement effective communication with staff, physicians and patients to ensure coordinated care and optimal patient outcomes.
• Plan and implement specific physical therapy treatment programs to aid recovery.
• Educate patients in proper care and use of supports and exercises.
• Follow documentation and reporting procedures.
• Collaborate with physicians to ensure comprehensive service delivery.
