We have an exciting opportunity for a Per Diem (PRN) Physical Therapist position, offering 10-15 hours per week on a consistent PRN basis.

We are seeking a PT to join our in-house team of dedicated professionals who provide person-centered care to our residents, aiming for positive outcomes.

Physical Therapist Responsibilities

• Consult with patients to understand their physical condition.
• Assess and interpret patient evaluations and test results.
• Develop treatment plans using various techniques.
• Create personalized rehabilitation programs.
• Administer prescribed physical therapy treatments to relieve pain, restore function, and promote healing.
• Ensure all treatments adhere to the established care plan.
• Monitor patient responses and modify treatments as appropriate in collaboration with the interdisciplinary team.
• Provide education and support to patients, families, and caregivers.
• Collaborate with team members to ensure quality care.
• Complete thorough and timely documentation in compliance with regulations.
• Attend meetings as required by the Director of Rehab.

Physical Therapist Skills

• Experience in SNF/Long-term care preferred.
• Current knowledge of treatment practices.
• Knowledge of Net Health Electronic Documentation System preferred.
• Ability to manage patients with diverse personalities.

Physical Therapist Requirements

• Active/Valid Texas Physical Therapy license.
