Job Position:

Physical Therapist for an In-house Skilled Nursing Facility

Job Type:

PRN positions available

Job Purpose:

To assist in planning, organizing, developing, and directing Physical Therapy services in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by the Administrator, to ensure the highest quality of resident care.

Job Functions:

1. Provide physical therapy services to residents according to established standards, procedures, and productivity standards.
2. Adhere to company procedures regarding documentation and billing of services.
3. Evaluate residents and develop treatment plans with short and long-term goals.
4. Conduct screenings and specialized evaluations as needed.
5. Communicate evaluation findings and treatment plans to residents, families, and team members.
6. Complete all required documentation in compliance with CMS expectations using NetHealth EMR systems.
7. Perform other duties as assigned to support organizational goals.

Education & Requirements:

1. Bachelor of Science Degree in Physical Therapy from an accredited program.
2. Active Physical Therapy license in the state of practice.
3. Ongoing continuing education to maintain licensure and certification.
