AW is a rapidly growing home health agency, with an innovative approach to patient centered care. Our team consists of over 500 employees serving over 1500 patients in Missouri and Illinois.
Job Summary:
We are looking for a part-time PT to service our Jefferson County community and Chesterfield / Ballwin areas. This individual will be seeing In Home Outpatient & Hospice patients in their place of residence.
An Physical Therapist (PT) administers physical therapy to patients on an intermittent basis in their place of residence, therapy clinic, or living facility. This is performed in accordance with physician orders and plan of care under the direction and supervision of the Regional Director of Therapy, Outpatient.
Responsibilities:
Work Environment:
Works indoors in Agency office, residential living facilities, and patient homes. Can travel to/from patient homes.
Job Relationships:
Supervised by: Regional Director of Therapy, Outpatient
Workers Supervised: Physical Therapy Assistant and Home Health Aide as assigned
