As a Physical Therapist at Enhance Rehabilitation, you will be part of a growing team providing high-quality care for patients in SNF settings. Your responsibilities will include evaluating and treating patients following individualized plans of care.

1. Providing excellent care for our patients.
2. Delivering excellent customer service to patients and family members.
3. Collaborating with facility staff to ensure optimal patient care.
4. Evaluating new patients, developing care plans, and setting measurable, functional goals.
5. Treating patients, addressing their concerns, and educating them on their injuries, progress, and probable outcomes.
6. Supervising Physical Therapist Assistants.
7. Re-evaluating patient progress, revising plans of care, and resetting goals.
8. Completing timely and accurate documentation, including evaluations, re-evaluations, discharges, and daily progress notes.
