Qualifications:

• A Bachelor's, Master’s, or Doctorate Degree in Physical Therapy from an Accredited Program
• Valid or pending State Physical Therapy License

Job Responsibilities include, but are not limited to:

• Evaluate, review, and screen patient/resident’s medical charts, evaluations under physician order, and develop a treatment plan according to patient’s needs
• Implement treatment plans through direct treatment and educate the team, including supervision of PTAs
• Attend/Lead interdisciplinary meetings, relay clinical findings, and make recommendations; conduct staff and family training as appropriate
