Job summary

Colonial Manor is a post-acute care facility in Randolph, NE, seeking a PRN Physical Therapist for our in-house rehab program. You will screen and evaluate patients, plan and deliver rehabilitative interventions, and contribute to wound care when needed.

Qualifications

• Physical Therapist license is required.
• This position is open to PTs at all experience levels.

Responsibilities

• Screen and evaluate patients with physical function disorders and wound care; develop care plans.
• Deliver rehabilitative, skilled, and medically necessary treatments.
• Administer procedures as part of the rehabilitation plan, including manual techniques, ambulation, exercises, modalities, devices, and wound care.
• Provide consultation and counseling to patients, families, caregivers, and other service providers.
• Generate discharge summaries compliant with regulatory and clinical requirements.
