Job summary

Horizon Home Health and Hospice, Meridian, ID, seeks a licensed Physical Therapist to provide patient-centered therapy in a home health setting. You will assess patient needs, develop plans of care, and guide families and caregivers in exercises and home treatments.

Responsibilities include direct therapy, priority assessment, progress documentation, and participation in discharge planning; prior home health/hospice experience is preferred and CPR/driver’s license are required.

Qualifications

• PT licensed in Idaho as a credential.
• Current CPR certification, valid driver’s license and auto insurance are required.
• Home Health and Hospice experience is preferred.

Responsibilities

• Provide physical therapy services per physician’s plan of care.
• Assess functional status, mobility, strength and ROM to inform the care plan.
• Direct physical therapy treatment and educate patients and families.
• Train caregivers in use of therapeutic devices and home health routines.
• Support discharge planning with progress summaries and goals.
