Horizon Home Health and Hospice Meridian, ID seeks a licensed Physical Therapist to provide therapy services per physician plan of care and to assess functional status and discharge needs. You will evaluate the home environment, direct therapy, and educate patients and families in therapeutic devices.

The role involves supervising PT Assistants, coordinating with caregivers, and identifying additional home health needs while maintaining documentation standards.

Qualifications

• PT Licensed in the State of Idaho
• Must have a current CPR, Driver's License and Auto Insurance
• Home Health and Hospice experience preferred

Responsabilités

• Provides physical therapy services to patients according to a written physician's plan of care.
• Evaluates home environment and assesses for muscle strength, mobility, gait, ROM and rehab potential.
• Directs physical therapy treatment and instructs patients and families in use of therapeutic appliances.
• Determines priority needs for physical therapy and reports patient reactions and condition changes to the physician.
• Initiates PT program and instructs other personnel and family members on goals of therapy.
• Identifies patient and family needs for other home health services and refers as necessary.
• Prepares and submits clinical progress summaries and discharge plans.
• Provides PT consultation to home families/caregivers and in-service education to staff.
