Job summary

Enhabit Home Health & Hospice is seeking a licensed Physical Therapist to provide care to patients in their homes across Idaho. You will assess, diagnose, and treat mobility impairments using evidence-based physical modalities while educating families and caregivers on home programs.

In this role, you will collaborate with physicians, nurses, and support staff to implement personalized plans of care. A valid Idaho PT license and reliable transportation are required.

Qualifications

• Must possess a valid state driver’s license.
• Must possess automobile liability insurance.
• Must wear seat belts at all times while driving.
• Must have dependable transportation kept in good working condition.
• Must be able to drive an automobile in all types of weather conditions.
• Must be currently licensed in the state of employment.

Responsibilities

• Examine and treat patients with physical impairments using physical modalities.
• Help patients improve mobility and independent self-care.
• Administer skilled care per physician orders and the established plan of care under supervision.
