Choice Health at Home in Tucson, AZ is seeking a PRN Physical Therapist to assess, plan, and provide rehabilitative services in patients' homes. You will develop individualized treatment plans, monitor progress, ensure infection-control practices, and document care in accordance with agency guidelines.

Competenze

• Accredited physical therapy program completed.
• Active state Physical Therapist license required.
• Minimum of one year home health experience preferred.
• Current CPR certification.
• Valid driver’s license and automobile insurance.
