Job summary

Choice Health at Home is seeking a PRN Physical Therapist to provide home health services in Safford, AZ. You will evaluate patients, design personalized treatment plans, and work with a multidisciplinary team to restore function and improve quality of life in patients' homes.

The role requires an accredited PT degree, valid PT license, CPR certification, and a valid driver's license. Prior home health and HomeCare HomeBase experience are preferred for this PRN position.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current Physical Therapist (PT) license in the state of practice.
• Current CPR certification.
• Valid driver's license, reliable transportation, and current auto insurance.
• One year of home health experience preferred.
• HomeCare HomeBase experience preferred.

Responsibilities

• Perform comprehensive physical therapy evaluations and establish individualized treatment plans.
• Develop and implement therapeutic interventions to improve mobility, strength, balance, and functional independence.
• Monitor patient progress, modify treatment plans as needed, and document outcomes.
• Educate patients, families, and caregivers on exercises, safety, and disease management.
• Collaborate with physicians, nurses, and the interdisciplinary care team to ensure coordinated patient care.
• Maintain timely, accurate, and compliant clinical documentation.
• Follow infection control practices and all agency policies and procedures.
• Promote exceptional patient care while supporting the mission and values of the agency.
