Careage Hills Rehabilitation in Cherokee, IA is seeking a PRN Physical Therapist to join our in-house rehab program. The role focuses on screening, evaluation, and implementing evidence-based treatment plans for geriatric patients.

Licensed Physical Therapists at all experience levels are welcome, including new grads. This PRN position offers alignment with our CAPLICO-valued team and opportunities within a post-acute setting.

• Licensed Physical Therapist required.

Responsabilités

• Screen and evaluate patients with physical functioning disorders and wound care; develop care plans per standards.
• Deliver rehabilitative, skilled, and medically necessary treatments.
• Administer procedures as part of the rehabilitation plan, including manual techniques, ambulation, exercises, modalities, devices, and wound care.
• Provide consultation and counseling to patients, families, caregivers, and other providers.
• Generate discharge summaries adhering to regulatory and clinical requirements.
