Hillcrest Health Care Center in Hawarden, IA, is seeking a PRN Physical Therapist to join our in-house rehab program serving geriatric patients. The role involves screening, evaluation, and delivering rehabilitative care to improve function and quality of life, with emphasis on wound care when needed.

Qualifications

• Physical Therapist license is required.
• New Grads are welcome and open to all experience levels.

Responsibilities

• Screen and evaluate patients with physical functioning disorders and develop care plans.
• Deliver rehabilitative treatment interventions in accordance with standards.
• Administer procedures as part of the rehabilitation plan including wound care and modalities.
• Provide counseling to patients, families, and caregivers regarding physical disorders.
• Generate discharge summaries meeting regulatory and clinical requirements.
