Qualifications

Responsibilities

• Conduct initial and follow-up evaluations to assess resident needs and progress.
• Develop and implement treatment plans to achieve optimal outcomes.
• Provide direct physical therapy services, including exercises, modalities, and other interventions.
• Monitor and document resident progress, making adjustments to treatment plans as needed.
• Participate in interdisciplinary care conferences to discuss resident care plans and progress.
• Maintain accurate and detailed records of patient progress and treatment plans.
