Job summary

Hireology is seeking a Per Diem Physical Therapist to support our in-house rehab team in Texas. You will evaluate patients, design and implement personalized rehab plans, and collaborate with an interdisciplinary team to achieve positive outcomes.

Active Texas PT license and good standing with regulatory boards are required. In-house rehab and flexible scheduling are highlighted benefits of this PRN role.

Qualifications

• SNF/Long term care experience preferred.
• Knowledge of current treatment practices.
• Knowledge of Net Health Electronic Documentation System preferred.
• Ability to manage patients with different types of personalities.

Responsibilities

• Consulting with patients to learn about their physical condition.
• Assessing and interpreting patient evaluations and test results.
• Developing treatment plans using a variety of treatment techniques.
• Creating personalized rehabilitation programs for patients.
• Administering medically prescribed physical therapy treatments to relieve pain, improve or restore function, and promote healing.
• Ensure all treatment delivered is in accordance with the established plan of care.
• Monitor patient response to treatment and modify approach with IDT collaboration.
• Provide clinical support and education to patients, family members, and caregivers.
• Collaborate with interdisciplinary team to ensure quality patient care.
• Ensure documentation is completed thoroughly and timely and complies with regulatory requirements.
• Attend required meetings as designated by the Director of Rehab.
