Job summary

Emerald Nursing & Rehab Columbus is seeking a PRN Physical Therapist to join our resident-centered rehab team. You will evaluate residents, develop personalized treatment plans, and deliver evidence-based interventions to improve mobility and independence in a supportive setting.

Requirements include graduation from an accredited PT program and current PT license. Experience in skilled nursing or long-term care is preferred; excellent communication and teamwork are essential.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current Physical Therapist (PT) license in good standing.
• Experience in skilled nursing, rehabilitation, or long-term care preferred.
• Excellent communication, interpersonal, and problem-solving skills.

• Ability to work collaboratively in a team-based environment.

Responsibilities

• Conduct comprehensive evaluations and create personalized treatment plans.
• Deliver evidence-based therapeutic interventions to improve mobility, strength, balance, and function.
• Monitor progress, adjust plans as needed, and document outcomes.
• Collaborate with nursing, physicians, and the interdisciplinary team to ensure holistic care.
• Educate residents and families on treatment strategies and goals.
• Maintain timely, accurate clinical documentation per regulatory standards.
